Health Information

We now need to ask several questions in regards to your health and the health of anyone else included in this form. health assessments form an integral part of any Visa application, so the more we know, the better. Please answer all of the following questions as accurately as possible.

In the fields below please list any conditions you have and your height and weight. You should provide details relating to any current or previous medical conditions requiring treatment of any description. This includes any learning or development issues.

Health Information

We now need to know which countries you or anyone included in this evaluation have spent time in. These questions can be a little confusing so read the following very carefully before you answer. We need to know which countries you or anyone included in this form have spent time in, under two different scenarios:

1) Firstly we need to know which countries you have spent 12 months or more in (whether in one visit or mutliple visits added together) within the last ten years.

2) Secondly we need to know which countries you or anyone in this form have spent 5 years or more in (whether in one visit or multiple visits added together) since turning 17 years of age.

Character Details

Like health, your character is given close scrutiny by the immigration authorities in both New Zealand and Australia so we need to make sure we obtain any relevant information from you and anyone included in terms of your character. There are two sections for each person:

1) Criminal History - if you or anyone included has ever been charged or convicted of any offence (whether expunged or not) in any country or if you are currently under investigation or wanted for questioning in relation to any criminal matter, we need the details.

2) Immigration Details - if you or anyone included here has ever been deported, refused/declined a visa, refused entry or removed from any country then we need the details.

English Details

English ability is an important part of almost all Visa applications for those 16 years of age and over. Please answer the following, giving an honest appraisal of each person's English ability. If you or anyone included has ever completed an English test, please provide the details below.

Family Support

For some Visa categories, having close family members living in Australia or New Zealand can assist in adding points or potential sponsorship opportunities. You can add up to four family members living in either country (Australia or New Zealand)

Financial Details

In most cases, finances are irrelevant to the question of whether you qualify to migrate to Australia or New Zealand and it is usually based on your skills or family ties, however we request this information to ensure that we are able to assess you against all possible options, including business or investment categories.
